I consent to being offered infant feeding support and information by Charlotte Hughes, International Board-Certified Lactation Consultant. This consent is for in person visits, phone conversations and appropriate information sent via email or text, which will include a summary report of the consultation (which can be shared with GPs and other healthcare professionals) and follow up information.

I understand that if applicable the consultation may include:

I understand that no medical advice will be offered.

I understand that if I have any concerns about myself or my baby that I should seek medical advice from a healthcare professional.

I give my consent for the lactation consultant to release any information acquired in the evaluation and/or management of myself and/or my child to our healthcare providers. I understand the lactation consultant may contact my GP or my child’s GP if the lactation consultant feels it is necessary.

I give consent for the lactation consultant to use anonymized clinical information obtained during our sessions for supervision and education purposes. I won’t be identified in any way, but aspects of my situation may be described and discussed.

I understand that for all consultations and follow up, the lactation consultant will protect the privacy of my personal health information as required by the Code of Ethics of the International Board of Lactation Consultant Examiners.

I understand that client data will be stored in accordance with GDPR 2018. Information requested will only be what is needed to provide services effectively and safely. No information will be used for marketing purposes. Clients have the right to request access to their personal data and the right to correction and deletion (where appropriate). Clients have the right to complain if they are concerned about how their data is managed.

Cancellation policy- Please let me know as soon as possible if you no longer require an appointment. Cancellations more than 48 hours before a booked appointment will be refunded. Cancellations within 48 hours of appointment- no refund but can be rescheduled by mutual agreement.
